Consumer Direct Services

Primary Home Care, LLC provides in-home services for you or your loved ones who need assistance to continue to live at home, due to a qualifying disability or health condition. Our staff is trained in and embraces the philosophy and practices of self-direction and the principles of person-centered planning.

We are a recognized leader in self-direction and one of the few human service agencies in the state that provides all of the consumer-directed service options available to persons interested in directing their own care.
